The  Diocese of Manchester's web site has many forms you may need, including:

• Volunteer Driver / Vehicle Registration form (look under Risk management, then Auto) - this form is needed if you are driving on a field trip or other school related event
• Permission slip for field trips (look under Risk management, then Liability)
